How a Centre Hastings Resident Applies for Long Term Care
Ontario uses one standardized provincial process for all LTC admissions. You cannot apply directly to the home—all applications go through Ontario Health atHome (formerly the LHIN / Home & Community Care).

Your Ontario Health atHome care coordinator begins the whole process.
They will:
- Confirm if you are eligible for LTC
- Assess your medical, functional, and safety needs (p. 3–4)
- Discuss whether you can stay safely at home while waiting
- Help you list your preferred LTC homes (Clare McFaul LTC will be added to the provincial system before opening)
You can self-refer, or a family member, doctor, nurse, or community support worker can refer on your behalf.
Once Clare McFaul LTC begins offering tours or publishes information, you can:
- Request an information package
- Review inspection and compliance expectations (once opened)
- Ask about room types and cultural/linguistic preferences
- Confirm available programs and the care model
- Understand room accommodation: private / semi-private / basic
Even before opening, you can still add the home to your list of choices once it appears in the provincial LTC database.
Your care coordinator will:
- Confirm your eligibility
- Assess your care needs
- Determine if LTC is appropriate vs. home care, supportive housing, retirement living
- Complete required medical and capacity-related assessments
- Ensure you (or your substitute decision-maker) give informed consent
- Complete the LTC application forms with you
You may choose up to 5 LTC homes at a time (ONLY LIST HOMES YOU WOULD ACTUALLY GO TO – IF THERE IS ONLY ONE HOME YOU WANT TO BE IN JUST LIST ONE)
Ontario Health atHome submits your application to the homes you selected, including Clare McFaul LTC.
- Each home reviews whether they can meet your care needs
- If accepted, you go onto their waitlist
- Wait times depend on:
- Room type (private, semi-private, basic)
- Your care needs
- Priority status
If this is a new home (opening 2026):
- Early applicants often get shorter wait times
- The home will still be required to follow the provincial first-available-bed and priority rules
When a bed becomes available:
- Your care coordinator calls you
- You have 24 hours to accept or refuse
- If you accept → You must move in within 5 days
- If you refuse while waiting at home → your entire application is closed for 12 weeks unless there are exceptional circumstances
If Clare McFaul LTC is your second or third choice, you may still remain on the list for your preferred LTC after moving in.
- Ontario Health Card
- Medication Administration Record or medication bottles
- Clothing that is machine-washable and labelled
- Assistive devices (walker, wheelchair)
- Toiletries
- Glasses, dentures, hearing aids (labelled)
Avoid bringing valuables or unsecured food items.

This does not guarantee priority, but your placement coordinator will consider proximity, caregiver distress, safety, and clinical need.
✔ You can apply BEFORE the home opens
As soon as Clare McFaul LTC appears in the Ontario LTC listings (usually several months before opening), you can add it to your 5-home list.
✔ Clare McFaul will likely have high demand
New builds typically have substantial early waitlist activity.
Applying early is strongly recommended.
✔ If you are in crisis or hospital
You may receive a priority status, which can shorten wait time.
